ADMISSIONS – Grades K – 8 (All PreK Gifted Information in next section, below)
Eastbank Collaborative Application Availability – October 8, 2012 at 8:30a.m. – January 11, 2013 at 2:45p.m. Applications are available for pick-up at Lake Forest or via the schools website.
Applicants may download the 2013 – 2014 admissions application by clicking on the application link. Before submitting the application, please ensure you have the required documents listed under the heading: Completed Applications Must Include TWO Copies of The Following. If you input your information online, the application WILL NOT SAVE; therefore, you must print the application after completion. Applicants are also encouraged to pick-up a complete application packet at Lake Forest Charter School located at 12000 Hayne Blvd. The deadline for a timely application is Friday, January 11, 2013 at 2:45p.m. Applicants are required to submit in person from 8:30a.m. to 2:45p.m. completed applications with all requested documentation to the school office located at 12000 Hayne Blvd.
Out of state potential residents (only) may submit an application packet via the United States Postal Service or an independent mail carrier. However, all mailed applications must be received by Lake Forest by Friday, January 11, 2013 at 2:45p.m. along with all required documents in order to be considered a timely application. All applicants are required to assess at Lake Forest Charter School.
Applications are considered late after 2:45p.m. on January 11, 2013. Lake Forest will accept late applications until September 30, 2013. Late applications are processed only if availability exists in the grade to which the applicant has applied and the waiting list has been exhausted.
Incomplete applications are not processed.
Admissions Process
Lake Forest Charter School accepts applications based upon availability. Prospective students are required to reside within Orleans Parish. Lake Forest Charter’s application process has been approved by the Office of Civil Rights. All applicants are required to complete and submit an application packet, complete an admissions assessment, attend a mandatory orientation meeting, and submit other required documentation within the allotted time frame.
Testing dates and times are provided upon application submission. All applicants are required to assess at Lake Forest.

ADMISSIONS – Pre-Kindergarten Preliminary Application
Our Pre-K Gifted class is open to children who meet the criteria as gifted according to LA Department of Education Bulletin 1508. Anyone interested in applying for one of 15 spots available must follow the process below. Once the 15 student limit has been met, others that qualify will be placed on a waiting list.
The child must be evaluated by a licensed psychologist. New Orleans Public Schools Exceptional Children’s Services offers this service at no charge to residents in the parish of Orleans. The office number is 304-4900, or parents may contact Dionne George at 359-8154. Please call to request an appointment to have your child evaluated for a Pre-K Gifted Program as soon as possible as their appointment calendar fills very quickly.
Once the evaluation has been completed, a report will be given. The report will indicate whether or not the child has met the criteria for gifted according to LA Dept. Of Ed. Bulletin 1508. If the report states that no exceptional services are needed at this time, the child has not met the criteria according to Bulletin 1508. If the child has met the criteria, a multi-page report will be received. A copy of the multi-page report must be submitted to Lake Forest upon receipt. Lake Forest cannot offer placement until the report is received.

2 COPIES of Birth Certificate
2 COPIES of Social Security Card
COPIES of 3 proofs of domicile in the parish of Orleans with the same parents/guardian’s name and address that is listed on the student’s application
2 COPIES of immunizations
3 Large letter size self-addressed, stamped envelopes
All information above should be turned in to Lake Forest in person so that the official application log can be signed.
